Procedure Steps

Receive application form after resident registration or download online

Fill out form (attach photo)

Visit city/ward office on reserved date to submit application

Receive notification card (about 1 month later, by mail)

Make pickup appointment and visit in person to receive card

Required Documents

  • Personal number notification card (received by mail upon entry)
  • Application form (download online or obtain at city hall)
  • ID photo (4.5cm×3.5cm)
  • ID documents (residence card, passport)

Important Notes

  • ⚠️ Application methods and processing time may vary by region
  • ⚠️ Reservation required before visit (cannot apply without reservation)
  • Online application available (My Number Portal, PC/smartphone)
  • Required for tax filing, health insurance, pension, banking, etc.
  • Takes about 1 month from application to issuance
  • If lost, immediately report to police and call center (0120-95-0178)
  • Free issuance (reissuance fee ¥1,000)
